To help parents meet the Oregon state requirement for a pre-participation medical exam for their middle or high school students, The Center Foundation offers no-cost, pre-participation sports exams every year in August. Supported by The Center and Therapeutic Associates Inc. medical professionals and volunteers come together to make sure every kid is qualified and healthy to play sports. Thousands of free student physicals have been conducted since the beginning of this community service.

Who Is Eligible?

Free pre-participation sports physicals are available to all students in grades 6-12. Insurance is not required.


Students are required to bring the form with a parent signature. The student medical history and parent signature must be completed before the exam. Click here to download the form.

Health and Wellness Fair

During the sports exams we host a youth Health and Wellness Fair offering information and resources related to healthy kids and families. Community businesses and groups participate with interactive displays, information, and activities. If you are interested in having a booth at this year’s health fair, please contact Andrew Traut at


Deschutes County Health Services offers teen immunizations including Tdap, HPV, and Meningococcal vaccines. Please bring a copy of your child’s immunization record. If you have insurance (including OHP), please bring your card. If no insurance, there will be no charge for vaccines.
